Pinnacle bosses
Do not fight these until you clear T13+ comfortably. In 0.5 the old Calamity Fragments were removed, and each pinnacle now has two versions: a one-time Quest version and a repeatable Infinite Farm version (via keys).
Apex
Arbiter of Divinity
- Unlock
- Place an Origin Core at the Origin Tower. Make it from an Origin Spark + Origin Cradle dropped by Phyx and Phya in the Matriarch / Patriarch Halls (T15+ Waystones).
- Drops
- Lightning-heavy. Killing it 5 times auto-completes Fortress sections. Drops 4 uniques: Decree of Acuity, Decree of Flight, Decree of Loyalty, The Ordained.
Gateway
Arbiter of Ash
- Unlock
- Demoted into the Fortress at the Burning Monolith. 3 Crisis Fragments (Ancient / Faded / Weathered) from the three citadels. Fortress Tier 6+, Enigma Chambers Tier 10+.
- Drops
- No longer the apex. Morior Invictus body, Prism of Belief, Sacred Flame.
Breach
Xesht
- Unlock
- Breachstone (made at the Genesis Tree) in the Realmgate. Difficulty 0 to 4; HP scales 6M up to 32M.
- Drops
- Hand of Wisdom and Action, Choir of the Storm, The Pandemonius. Each tier's first kill grants Breach Atlas points.
Delirium
Tangmazu, the Raven Trickster

- Simulacrum (7 waves) at 100% deliriousness, then the Raven's Reflection key. New 0.5 Delirium pinnacle.

- Signature drop: the Voices unique jewel.

- Rite of the Nameless: chain 5 maps into one continuous Ritual. New 0.5 Ritual pinnacle.

- King in the Mists / Queen in the Mists are encounters here, not the pinnacle. Chase: An Audience with the King, Ingenuity belt.

- Floor 4 of Trial of the Sekhemas, Djinn Barya area level 75+. Exempt from Atlas-tree difficulty scaling.

- Temporalis (no-hit run with The Last Flame relic), Sandstorm Visage.

- Trial of Chaos, last 2 ascendancy points. Also exempt from Atlas-tree scaling.

Expedition
Ocean faction leaders

- Medved, Vorana, Uhtred, Olroth on the new ocean islands (tied to Expedition / Runes of Aldur). Olroth gates the Runes of Aldur pinnacle.

- Olroth: Keeper of the Arc, Olrovasara, Svalinn, Heroic Tragedy, Olroth's Resolve. The Runes of Aldur pinnacle is not officially named yet.
Verify in-game
Exact boss HP for the Arbiter Quest vs Infinite Farm versions, and whether Melting Maelstrom still drops from the 7-wave Simulacrum or from Tangmazu. There is no “Nameless King” boss in PoE 2.
